DEVIC'S SYNDROME + TRANVERSE MYELITIES

Efek dari pnyakit ini adalah :

  1. lumpuh separuh badan

What is Devic's Syndrome?

Devic's syndrome is a rare autoimmune central nervous system disorder characterized by transverse myelitis (in which the fatty, protective covering of the spinal cord breaks down) and optic neuritis (in which inflammation of the optic nerve causes loss of vision and eye pain).]. It is considered a special form of multiple sclerosis (MS) with a severe and rapid course. The disorder affects the optic nerve and the nerves in the spinal cord. In Devic's syndrome, the fatty sheath that protects these nerves is lost. Individuals may experience vision impairment and various degrees of paralysis, as well as incontinence. The disorder is closely linked with MS and lupus, but usually appears before any symptoms of MS are noted. If an isolated disease episode affecting the spinal cord and optic nerve occurs after an infection or common cold, it is considered a post-infectious acute demyelinated encephalomyelitis (ADE) rather than Devic's syndrome.

Is there any treatment?

There is currently no standard treatment for Devic's syndrome. Generally, treatment is symptomatic and supportive. Corticosteroids may be prescribed. Treatment for ADE may include corticosteroids, intravenous immunoglobulin, and intravenous methylprednisolone

What is the prognosis?

Devic's syndrome is fatal in many patients. Some ADE patients achieve complete or nearly complete recovery while others may have residual deficits. Some severe cases of ADE may be fatal.

What research is being done?

The NINDS supports an extensive research program of basic studies to increase understanding of how the nervous system works. A major goal of this research is to develop methods for repairing damaged nerves and restoring full use and strength to injured areas.

HOW TRANSVERSE MYELITIS PROGRESSES:

  • back pain & pain around the trunk of the body-like a tight belt
  • tingling, numbness, burning, prickly sensations in limbs
  • weak leg mobility to varying degrees-including possible paralysis
  • weakness in the arms-like a heaviness leading to la ck of coordination
  • notably lowered sensory abilities in extremities (to various hot/cold temperatures & pain levels as tested by pinpricks)
  • lack of control over bladder and bowel movements

Causes of Transverse Myelopathy and Myelitis

Transverse myelitis may occur in isolation or in the setting of another illness. When it occurs without apparent underlying cause, it is referred to as idiopathic. Idiopathic transverse myelitis is assumed to be a result of abnormal activation of the immune system against the spinal cord. A list of illnesses associated with TM includes:

Table: Diseases Associated with Transverse Myelitis

Parainfectious (occurring at the time of and in association with an acute infection or an episode of infection).

Viral: herpes simplex, herpes zoster, cytomegalovirus, Epstein-Barr virus, enteroviruses (poliomyelitis, Coxsackie virus, echovirus), human T-cell, leukemia virus, human immunodeficiency virus, influenza, rabies

Bacterial: Mycoplasma pneumoniae, Lyme borreliosis, syphilis, tuberculosis

Postvaccinal (rabies, cowpox)

Systemic autoimmune disease

Systemic lupus erythematosis

Sjogren's syndrome

Sarcoidosis

Multiple Sclerosis

Paraneoplastic syndrome

Vascular

Thrombosis of spinal arteries

Vasculitis secondary to heroin abuse

Spinal arterio-venous malformation

Spinal Cord Diseases

The spinal cord can be damaged in many ways, producing various symptoms, depending on the location, or level, of spinal cord damage. The spinal cord can be cut in an accident, compressed, destroyed by infection, damaged when its blood supply is cut off, or affected by diseases (such as spinal cord cysts, cervical spondylosis, or multiple sclerosis) that alter its nerve function.
